The Scottish Government has issued an update on the use of the supplementary form for cremation applications. The form was introduced in April 2020 to be used by funeral directors when it was impossible for them to get the signature of the applicant. It is not intended to be the default position for all applications, and should not be used for the convenience of funeral directors. If challenged, a funeral director who uses the supplementary form would need to justify why they were unable to receive the signature of the applicant. Further information can be found here.
